You can get cheap ones, but well, nothing like the feeling that it was done with your own hands.

I haven't epoxyed the fron bezel to the CD-ROM's casing I got for it yet.
It's because I have soldered some more things to the PCB that need to be finished, but I don't have enough time right now.
That's why it isn't straight.

I've bought black and silver knobs, and added a switch and a headpone jack, with the swicth I select the output ,headpones or speaker. The two buttons on the right are the volume control for my SB Live!

I cannot really tell you since I live in Uruguay.
Prices are in the guide, but those prices were in RadioShack, so you can get all those parts cheaper somewhere else.
It also depends on how many controllers you want, I did 6, all of them are already used, but you can have more than one fan per controller since it can handle up to 1.5A in variable mode.
